State of Massachusetts Forensic Transition Team Supervisor in Boston, Massachusetts

The Department of Mental Health (DMH) is seeking qualified applicants for the position of Forensic Transition Team Supervisor. Under the supervision of an Area Forensic Director, the Forensic Transition Team (FTT) Supervisor will be responsible for specific operations of the FTT program within statewide FTT protocols especially focused on institutions and clients in the assigned Areas. The Supervisor will be responsible for clinical & administrative supervision of FTT positions, ensuring cross-Area integration of the FTT program and maintaining strong ties to local DMH services sites and community providers. The Supervisor will also directly provide transitional services for individuals with serious mental illness who are involved with courts, jails, prisons, parole, probation, or in certain situations, inpatient units. Services include assessment, release planning, outreach, consultation/liaison, coordination of recommended services, advocacy, and data entry as required. // /_Duties and Responsibilities (these duties are a general summary and not all inclusive):_/ * In consultation with the Area Forensic Director, develops, coordinates and evaluates Area-specific programming for linking persons with serious mental illness to appropriate services upon release from incarceration through collaboration with administrators and service providers involved in state and country corrections, DYS, Parole and community-based mental health services, and emphasizing continuity of care from the local Houses of Correction and DOC facilities. * Prepares Area-specific reports and the request of the Area Forensic Director and/or the Assistant Commissioner for Forensic Services regarding the operations of the release planning efforts, including outcome review of those involved with FTT services. * Supervises assigned staff including: a) planning and assigning work according to the nature of the job, the capabilities of the subordinate staff and the available resources; b) determining subordinate staff training needs and arranging for such training; c) motivating subordinate staff to work effectively; and d) determining the need for corrective action and making appropriate recommendations for such to higher level management staff. * Consults with state and county corrections clinical staff, DYS and Parole to facilitate timely identification of, and eligibility determination for, potential DMH clients. * Works with members of the DMH service authorization team, conducting face-to-face clinical interviews in places of incarceration, providing any policy-related risk documentation, and working with the local DMH site offices. In collaboration with Site staff, may be required to write up Needs and Means assessments and present cases for DMH Site Office service team meetings to recommend needed services available only through the DMH referral system. * Coordinates interagency involvement and advocates with relevant service providers (e.g., corrections, court clients, Parole and Probation and insurers) to facilitate service provision to DMH clients. Works in tandem with the Area Forensic Director, the court, and corrections around jail diversion efforts. * Provides service coordination and discharge planning for forensically involved clients plus short-term outreach services to encourage successful community reintegration. * Develops a helping and supportive relationship with clients during incarceration/hospitalization in order to encourage adherence to healthy, independent, quality lifestyle upon return to the community. Provides guidance to supervisees to do the same. * Links client to appropriate agencies and organizations that meet the client’s identified needs. Participates as a part of the client’s treatment team, as necessary. * Assists in compiling, evaluating, and documenting clinical and forensic data regarding clients identified as high risk, including those with Mental Illness and Problematic Sexual Behavior (MI/PSB). * Provides sound clinical assessments and evaluations of forensically involved individuals and presents relevant clinical information orally and in writing. Under direction from the Area Forensic Director, works with the state MI/PSB Program Director on clinical and other issues relevant to MI/PSB clients. * Develops expertise on the issues of homelessness and housing for high-risk clients; develops and maintains working relationships with staff involved in homeless and housing issues. _Required Qualifications:_ * Knowledge of diagnostic and treatment criteria for individuals with long term, serious mental illness. * Knowledge of social, emotional and experiential causes of social dysfunction. * Ability to work collaboratively with other mental health professionals. * Ability to think and operate on multiple levels regarding the planning for and delivery of services, including community and correctional mental health. * Knowledge of community resources as they relate to release planning. * Knowledge of the criminal justice system and state and county corrections, DYS and Parole Operations. * Knowledge of jail diversion and knowledge of applicable sections of M.G.L. Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required. In addition applicants must have at least 5 (five) years of professional experience in the human service field of which one (1) years must have been in a supervisory capacity. Based on assignment, travel may be required.
