We are seeking a Licensed Practical Nurse (LPN) Case Manager  to join our team in our Bethlehem, PA location. The LPN Case Manager will be  responsible for: coordinating and implementing services for consumers as identified in the service plan. coordinating the overall medical care for consumers, following through with Doctor's medication orders. provide education for better understanding of medical conditions, advocacy during medical appointments. providing supportive counseling, training, and case management services utilizing a flexible schedule on-call responsibilities. Requirements

The ideal candidate will possess the following qualifications: Must be a Registered Nurse, Licensed Practical Nurse or bachelor's degree in nursing with a minimum of 12 credit hours in social science and a minimum of 2 years of experience in a mental health setting. Valid PA Driver's License other appropriate state and federal clearances.
