Dignity Health Medical Group - Sequoia is seeking an experienced , energetic and team oriented Primary Care (IM or FM) Advanced Practice Provider (PA or NP) in Belmont, CA .

Compensation/Benefits Include:
Compensation Range*: $120,000 - $140,000/year
Sign on, productivity and quality bonuses

Responsibilities
Patient Assessment and Diagnosis:
Conduct comprehensive health assessments.
Diagnose and treat acute and chronic illnesses.
Order and interpret diagnostic tests.
Treatment Planning and Management:
Develop and implement patient care plans.
Prescribe medications and therapies.
Provide health promotion and disease prevention education.
Collaboration and Consultation:
Collaborate with other healthcare professionals.
Consult with physicians and specialists as needed.
Coordinate patient care across various settings.
Qualifications
Candidates must have at least 2 years of experience as a Nurse Practitioner or Physician Assistant.
A valid and unrestricted license to practice in the State of California
Nationally recognized certification (as determined by Medicare)
Completion of state mandated educational courses
Pay Range
$57.69 - $67.30 /hour
