**[SOCIAL WORKER

[PRIMARY FUNCTION: [ To provided supportive medical social services and counseling to terminally ill patients and their families in accordance with an appropriate plan of care. Social Worker will assume responsibility for the development, implementation and management of psychosocial services to hospice patients and their families. *[QUALIFICATIONS:]{new="" roman="" times=""}]{new="" roman="" times=""}]{new="" roman="" times=""}]{new="" roman="" times=""}]{new="" roman="" times=""}*

  1. Have a Bachelor's Degree in social work from an accredited school of Social Work. A Master's Degree in social work is preferred. A person with a Bachelor's or advanced degree in psychology, counseling, psychiatric nursing, or other appropriate education and experience may also qualify for this position.
  2. One year experience in direct service as a Social Worker, i.e. nursing home, home health, however hospice is preferred.
  3. Experience in counseling with individuals, families and groups preferred.
  4. Ability to teach and speak publicly desired.
  5. ADEC certified and SW licensure preferred.
  6. Ability to access and utilize community resources.
  7. Documentation and computer skills.
  8. Valid N.C. Driver's License; reliable vehicle with valid insurance.
  9. Ability to work independently; planning, teaching, supervision.
  10. Belief and knowledge in hospice philosophy/concepts
  11. Strong communication skills, verbally and writing.
  12. Knowledge and understanding of grieving process.

**[JOB RELATIONSHIP: [ Supervised by the Medical Director.

[MAJOR RESPONSIBILITIES:]{new="" roman="" times=""}]{new="" roman="" times=""}]{new="" roman="" times=""}**

Make initial psychosocial assessment and plan of care.

Directly oversees bereavement programs.

Assess patient/family social and emotional factors and coping abilities on a regular basis.

Maintain confidentiality.

Provide psychosocial counseling and education to patients and families around death/dying, grief, financial, legal issues.

Assist in coordination of plan of care accessing community resources, assignments/supervision of volunteer.

Provide crisis intervention counseling, consult and collaborate with IDT.

Participate in all IDT meetings, in person or via phone, to establish, review and revise plan of care for each patient/family unit.
