Preferred Experience

  • Leadership experience with strong supervisory and management skills

  • Experience with case management and public health practice with a focus on social determinants of health

  • Computer literacy and familiarity with MS Office products as well as electronic medical records and statistical analysis software preferred

  • Familiarity with medical terminology is preferred

  • Adherence to HIPAA confidentiality requirements

Minimum Qualifications

MINIMUM QUALIFICATIONS:

Seven years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

Six years of full-time work experience in any field plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

A bachelor’s degree plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

An advanced degree or a Juris Doctorate plus three years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

EXPERIENCE SUBSTITUTION:

Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Examples of Work

The Population Health Manager/Community HealthWays Program Manager is responsible for supporting community health workers (CHW) by overseeing a team of approximately 30 community health workers who coordinate support for patients in need of health providers, care teams, and navigation. The position requires logistics, training, partnership development, reporting, liaising with intra- and inter-agency teams, and both analyzing and visualizing program data.

Primary Duties and Responsibilities

Manager

  • Lead a team of CHWs who will work with hundreds of patients each year

  • Support and coach CHWs so they do their best work for patients

  • Monitor caseloads of all CHWs, analyzing reports, monitoring client

  • enrollment timeline, progress towards goals, and CHWs’ support of clients

  • Organize and implement CHW training

  • Integrate CHWs with care teams working in clinics

  • Coordinate with providers and/or care teams to create, review, and triage patients who need to engage with CHWs

  • Facilitate team meetings and support groups for patients

  • Conduct basic accounting for project expenses and program management

  • Meet weekly with core CHWs to review caseloads and clinical coordination with providers

  • Coordinate special community events such as health assessments, mobile clinics, or other services as determined by CHWs, communities, and other stakeholders

  • Participate in on-going quality improvement (QI) activities within the roles defined above

  • Other duties as reasonably assigned Coordinator Analyze health system data to identify patients who are at high-risk for poor outcomes

  • Meet with eligible patients to enroll them in the program

  • Administrate activities including assistance with CHW recruitment, hiring and training

  • Collect data for program evaluation

  • Other duties as reasonably assigned
