Under the supervision of the Hospice Coordinator, the Social Worker provides short term therapy and treatment to patients and families with emotional, social and environmental issues related to or impacting the patient's illness or recovery. Assists the patient and family in planning for and securing adequate community resources for the patient's support, provides assistance with long range planning and makes referrals as necessary. Is a core member of the Hospice Interdisciplinary Group, provides support to Hospice Volunteers, and is responsible for the oversight of the Hospice Bereavement Program.

Applicant must hold a Master's Degree in Social Work from an accredited or approved school by Council on Social Education.

Current licensure to practice clinical social work in the State of Maine.

Must have a valid State of Maine Driver's license and show proof of auto insurance.
