Summary:

The Adult/Child Ambulatory Therapy Clinician is independently licensed (LCSW preferred) with experience in the treatment of mental health, addiction, or co-occurring disorders. Under the supervision of the Clinical Supervisor Child/Adult Services they are responsible for ensuring high quality, recover-oriented active treatment to assigned caseload through assessment, evaluation, treatment and discharge planning in accordance with regulatory policies. As direct care providers they function as skilled advocates, brokers, educators and treatment specialists and have demonstrated competencies at all levels of care within the hospital. They are responsible for development of multidisciplinary treatment plans that are based on the clinical assessment and data from formal and informal community support systems. They are familiar with regional social service provider systems and adept at assisting clients in linking with step down treatment services during discharge planning. They are proficient at ensuring that ongoing liaison occurs with families and referring community providers including primary care providers. They rely on more than one theoretical framework in providing direct client care or clinical formulations, are adept at articulating case reviews and creating documentation that reflects both active treatment provisions and client response to care. Therapy Clinician's act as role models for other staff members and also provide a professional service as leaders in teaching other non-professional staff. This person performs duties and/or responsibilities as assigned by the Manager or Supervisor for which s/he is qualified.

Responsibilities:

  • Completes a comprehensive, recovery-oriented bio-psycho-social evaluation (including five axis diagnoses, mental status exam, and case formulation) by collecting data which enables the social worker to identify potential therapeutic targets for active treatment in collaboration with the client within four hours of the initial contact.

  • Develops initial and updated service plans in collaboration with client and treatment team as treatment needs change, or within established time frames, to identify relevant recovery oriented goals and objectives for active treatment.

  • The master treatment plan is updated according to regulatory regulations.

  • The master treatment plan identifies effective interventions to achieve outcomes, patient's motivation, health/spiritual beliefs and functional capabilities and strengths.

  • The master treatment plan reflects a continuum of services to be provided.

  • The master treatment plan reflects the clinicians' ability to analyze, classify, and interpret social and personal data, including trauma assessments and initial evaluation of risk to self and others.

  • Implements the interventions identified in the master treatment plan using a wide range of interventions designed to prevent mental and physical illness, and promote, maintain and restore mental and physical health. Interventions are within the scope of practice of the clinician and may include individual/group/family therapy, cognitive behavioral therapies, EMDR, and Deep counseling that are directed towards maintaining and improving the patient's functional status and quality of life.

  • Regularly considers and involves ancillary services within active treatment.

  • Provides individual and group clinical supervision to other staff as assigned, ie. nursing staff, support and ancillary staff.

  • Mentors all new staff to ensure high quality services is provided.

  • Provides clinical supervision to conditionally licensed providers as requested by clinical leadership.

  • Implements interventions that are consistent with master treatment plan and developmental level and needs of client to elicit therapeutic change.

  • Interventions are modified based on continued assessment of the patient's response to treatment.

  • Interventions are performed according to the clinician's level of education and practice.

  • Completes annual assessment according to hospital policy.

  • Obtains a physician order for outpatient services per policy standards

  • Completes documentation within 24 hours and complies with regulatory standards as measured by chart review.

  • Completes documentation that demonstrates active client involvement in treatment.

  • Completes documentation that reflects barriers to treatment progress.

  • Expected treatment outcomes are patient oriented, evidence based, therapeutically sound, realistic and attainable and measurable.

  • Demonstrates reflective/active listening, feedback, summarizing, reframing, empathy, support, problem solving, coaching, and psycho-education skills.

  • Maintains productivity set by the Administrator of Adult/Child Ambulatory Services and Director of Social Services.

  • Maintains active treatment to assigned caseload if transferred to another level of care within the department as requested by Clinical Supervisor.

  • Consistently complies with the policies and procedures as set forth in the Clinical Policy and Procedure Manual.

  • Maintains and abides by ethical standards set forth by the facility and Maine State Licensing Boards.

  • Maintains requirements of licensure.

  • Fosters the development of teamwork within department as well as across departments and with outside providers.

  • Serves as a role model by developing respect, confidence, cooperation and participation in the work environment.

  • Reviews all meeting minutes.

  • Gives and receives critical feedback on effectiveness of clinical interventions through regular peer review, scheduled staff meetings and supervisions.

  • Follows through with commitments to the patients and other providers in a timely and professional manner.

  • Communicates plans and coordinates with other team members the scheduling of meetings with patients.

  • Utilizes time effectively and can critically evaluate time management relevant to the effective operation of respective department.

  • Provides feedback from all disciplines when evaluating, planning and implementing interventions.

  • Ensures and supports a therapeutic milieu through patient and family advocacy.

  • Provides direct service as needed in all hospital hotspots.

  • Assist in maintaining a work environment free from recognized hazards that create a risk of injury to employees, patients or visitors and that all accidents and incidents are reported by employees and properly investigated. When appropriate, assist with the return of all workers with work related injuries and illnesses to gainful employment.

  • Clinical documentation demonstrates use of relevant outcome data to inform clinical practice (including but not limited to OQ data).
