Job Title: Tele-psychiatry Healthcare Coordinator

Job Summary:

Key Responsibilities:

  • Supports the Integrated Tele-Psychiatry Team through collaboration with the program manager by assisting the leader of the psychiatry team. Duties such as attending and organizing critical meetings, keeping minutes, and scheduling invited guests.

  • Facilitates coaching at each of the designated clinics and advocates clinical leadership on tele-psych workflows. Coordinates with clinics to assure that clinic staff are prepared to assist provider and patient during clinic session.

  • Serves as the primary contact for trouble shooting scheduling and other workflow issues for the psychiatry team seeing patients at the various clinics.

  • Supports and initiates efforts for the ongoing quality improvement initiatives of the tele-psych service. This can include regular educational meetings, check-ins, and updating metrics in program dashboards.

  • Patient Outreach such as tracking patient appointment statuses; completed appointments, cancellations, and no-show visits. Assists with scheduling/rescheduling the cancelled and no-show visits as needed. Providing follow-up visit reminders to patients.

  • Support rooming process for tele-psych encounter by confirming the technology is working, patients can connect to the visit, documents patient is in Colorado for telehealth visit and assures screening scales are completed.

  • Support the follow-up of visits, include collecting collateral medical records, organizing & scanning these into the EHR.

Minimum Qualifications:

  • A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.

  • A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.

  • One (1) year of professional level experience in healthcare and/or clinic operations support.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:

  • Experience using Electronic Health Record and Patient Health Information management systems.

  • Experience with Epic.

  • Project management experience.

The Telepsychiatry Healthcare Coordinator will support the psychiatry team integrating behavioral health into primary care in support of the quintuple aim of enhancing the patient experience, improving health services provided to patients, health equity, reducing costs of care using best practices, and improving the care team well-being. They will work directly with the director of the integrated psychiatry team and the psychiatry team members in collaboration with primary care clinic leadership, staff, and providers to coordinate and execute the integration of tele-psych workflows in all designated family medicine clinics. This position may support rooming of patient visits, scheduling, following up as needed, and troubleshooting workflow issues independently. This position works closely with the program manager and the integrated Care team at the clinical level including psychiatrists, primary care providers, psychologists, and support staff at over 8 separate clinic locations. The intermediate professional will support the administrative leadership by assisting with creating tables and reports of month and annual patient metrics (number of visits, documenting time from referral to first visit, etc.). This position will also require some administrative duties such as independent maintenance of records, emergency binders, and attendance in meetings as record keeper. They may also be responsible for gathering information such as "lives touched" and work with the business intelligence collaborators in the department.

Hybrid – This role is eligible for a hybrid schedule of 3-4 days per week on campus and as needed for in-person meetings.

The starting salary range (or hiring range) for this position has been established as $61,546 – $78,287.
