Job Summary:

Key Responsibilities in Population Health (60% time):

  • Provide centralized population health outreach to address care gaps important to value-based programs, potentially including but not limited to:

  • Analyze population data to identify value opportunities related to cost or quality

  • Assist the Director of Population Health Pharmacy with design, evaluation, and transitions to "maintenance phase" (incorporation into routine clinical practice as appropriate) of existing decentralized population health efforts by the CU Primary Care Pharmacist Team.

  • Develop new quality improvement interventions directed at healthcare priorities established by OVBP + primary care leadership, potentially including but not limited to:

Key Responsibilities in clinical practice in primary care (40% time):

  • Provide clinical pharmacy services as a member of multidisciplinary team(s) at the Depot Hill clinic including direct patient care clinical that focus on chronic disease management using collaborative drug therapy management protocols.

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.

  • Applicants must have a PharmD degree

  • Completed an ASHP-accredited PGY2 residency training in ambulatory care, or a similar program

  • Three years' experience in ambulatory care and/or population health care

  • Candidates must have or be eligible for pharmacist licensure in Colorado.

Preferred Qualifications:

  • Previous clinical experience in ambulatory care at an academic medical center

This faculty member in the Department of Clinical Pharmacy will have two primary responsibilities: participation in collaborative systems-level population health medication management and quality improvement initiatives in partnership with University of Colorado Medicine (CU Medicine); and as a clinical pharmacist in a CU Medicine primary care clinic. CU Medicine is the region's largest and most comprehensive multispecialty physician group practice. The CU Medicine team delivers business operations, revenue cycle and administrative services to support the patients of over 3,500 University of Colorado School of Medicine physicians and advanced practice providers. The Office of Value-Based Performance (OVBP) obtains and manages value-based programs including business operations, quality improvement outreach, and quality improvement leadership, innovation, and evaluation. In the population health role (60% time), this faculty member will serve within OVPB under the Director of Population Health Pharmacy as part of a multidisciplinary team which operates from a strategic perspective to develop innovative programs and processes to improve the quality of healthcare across University of Colorado School of Medicine affiliated clinics. This position will have responsibilities related to value-based health care initiatives which improve the quality of care and value to the overall health care system. This position will also support value-based initiatives involving other clinical pharmacist faculty members providing direct patient care in CU Medicine/UC Health Family Medicine, Internal Medicine, and other ambulatory care clinics. This work may be performed via a hybrid model with a mix of in-office and remote work. As an ambulatory care pharmacist (40% time), this faculty member will provide clinical pharmacy services as a member of an interdisciplinary primary care team at the CU Medicine Family Medicine Depot Hill clinic, an expanding practice that recently located to a new larger location in Louisville, CO. Primary clinical responsibilities include developing and providing direct patient care clinical pharmacy services that focus on chronic disease management using collaborative drug therapy management protocols. Successful candidates will also interface with clinic providers to provide comprehensive medication management and other population health initiatives for patients. Clinical services and provider support will be delivered in-person for at least the first six months, with potential for a hybrid model thereafter. Opportunities for engaging in experiential education of pharmacy students and PGY2 ambulatory care residents, and for participating in scholarship in support of clinical and teaching efforts are available and encouraged. o Medication Adherence program o Statin use in persons with diabetes o Statin therapy for patients with cardiovascular disease o Osteoporosis management in women who had a fracture o Medication reconciliation post-discharge o Flu vaccine o Care for Older Adults - Medication review o Medication cost-reduction opportunities o This includes programs done in collaboration with the Ambulatory Health Promotions team (e.g. Fracture Liaison service, Hypertension/PharmD outreach program, gout medication optimization) o Reducing inappropriate ED visits (potential specific strategies below) § Optimizing treatment of migraine o Antidepressant follow-up to improve therapy persistence post initiation o Improving hypertension management § home BP monitor loaner program o Support development of new collaborative drug therapy management protocols as needed to support value-based care o Function as an interdisciplinary member of the primary care teams to support all medication management activities. o Establish collaborative working relationships with clinic providers and staff to become a valued and contributing team member. o Work with clinic providers, staff, and administration to develop and implement clinic- and population-level medication interventions targeted to improve quality metrics. o Work collaboratively with providers, clinic staff, and care managers to reduce adverse drug events and improve medication management for care transitions and complex or high utilizing patients. o Educate staff and providers regarding medication management best practices including updates on new medications and clinical guidelines, chronic disease state management, and others. o Provide quality teaching services in the School of Pharmacy Doctor of Pharmacy program. Teaching includes precepting student Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs), evaluating student performance using various assessment methods, and mentoring and advising students. Opportunities to prepare and present lectures and to facilitate active learning sessions are also available. o Produce collaborative scholarly work in support of clinical practice and/or educational activities. Scholarly activity includes publishing manuscripts (e.g., research articles, educational innovations, textbook chapters, therapeutic summaries/review articles) in peer-reviewed journals. Scholarly work may also include conducting research projects that are focused on demonstrating clinical innovations and demonstration of value of clinical pharmacy services. o Participate in professional service activities such as university or school committee work, participation in professional organizations, and attendance at seminars, conferences, special courses, and scientific meetings.

The starting salary range () for this position has been established as Assistant Professor: $125,000-133,000, depending on previous experience. Associate Professor: $145,000-151,000.
