Medication Safety Pharmacist

Job Description

Purpose & Scope:

The Medication Safety Pharmacist is responsible for the management and coordination of safe medication practices to ensure the safety, quality, and effectiveness of medication use throughout the pharmacy department and hospital. The responsibilities will include clinical informatics to monitor and manage medication safety metrics from the automation employed for the distribution of medications, the electronic medical record for medication builds, order panel and order set builds, data collection, medication documentation for revenue monitoring and accountability of medications as a means to evaluate pharmacy and hospital medication workflows , identify performance opportunities and mitigate high-risk medication practices. The pharmacist will be responsible for conducting safety assessments and medication error/adverse drug event analysis to improve patient outcomes with follow up reporting of safety metrics to the Medication Safety Committee and Pharmacy & Therapeutics Committee. The pharmacist role serves to provide effective communication among departments regarding safe medication practice, consistent with the hospital’s mission to provide high level quality and safe patient care.

Education:

Graduation from an accredited school of pharmacy is required.

Experience:

Three years of hospital pharmacy experience is required.

Infusion Pump library build and reporting experience is preferred.

Three years of Epic experience with medication builds, order panel and order set builds.

Demonstrated leadership ability is required.

Certification/Licensure:

Licensed as a Pharmacist by the Virginia Department of Health Professions required.

Epic Willow and/or Beacon analyst certification is preferred.

Other Qualifications

Medication Safety Certification to be completed within 12 months.
