What You’ll Do

The successful candidate will manage caseloads of enrolled patients including patient assessment/education via telephone, and/or face-to-face for coordination of care, establishing patient-centered care plans, and transition planning across the continuum of care. Provide education and care management for members enrolled in the care management and disease management programs to achieve: (a) desirable/acceptable outcomes/responses, (b) perceptions of satisfaction or acceptance of those involved. Support the Collaborative Care Management Model: Collaborating with physicians, social workers, pharmacists, and other professional staff to improve overall patient outcomes. Participate in interdisciplinary team conferences.

What You’ll Need

  • Associate's Degree Nursing is required.

  • Bachelor's Degree in Nursing is preferred.

  • Three (3) years of managed care and/or acute clinical care experience is required or three (3) years of case management experience is required.

  • Three (3) years disease management experience is required or Utilization Management and pre-authorization experience is preferred.

  • RN - Registered Nurse is required upon hire.

  • CCM - Certified Case Manager is preferred within 2 years of hire is preferred.

  • ACM - Accredited Case Manager within 2 years is preferred.
