Job Summary

Job Summary/Overview:

The Promotion of Health Equity (PHE) and Obstetrics Initiative (OBI) teams are a hybrid workforce. Remote work is supported and expected as appropriate within one's role. However, in person work is required to support community outreach, engagement, and other team meetings. We have two in-person meetings that vary in location, and approximately 1-2 meetings in person per month which will be in addition to the community engagement and outreach.

Summary

Promotion of Health Equity (PHE): The PHE project, with a Medicaid health information technology and exchange match grant from the U.S. Centers for Medicare and Medicaid Services (CMS), supports the Michigan Department of Health and Human Services (MDHHS) social determinants of health strategy. PHE was launched in 2020 and the main project partners include the Michigan Health Information Network (MiHIN), Collaborative Quality Initiatives (CQIs), and the Center for Health and Research Transformation (CHRT). We are all working together to develop sustainable, turnkey systems that allow physicians and other service providers to address the healthcare disparities and social drivers of health (SDOH) that disproportionately diminish the health of vulnerable communities across Michigan, particularly Medicaid beneficiaries.

The Obstetric Initiative (https://www.obstetricsinitiative.org/) (OBI): OBI is a statewide quality improvement initiative that collects clinically credible data and shares best practices across over 70 hospitals, with a goal of optimizing maternity care experiences, health outcomes, and equity for Michigan families. Support for the Obstetrics Initiative is provided by Blue Cross Blue Shield of Michigan and Blue Care Network as part of the BCBSM Value Partnerships program. OBI also participates in the Promotion of Health Equity (PHE) Grant funded by the Michigan Department of Health and Human Services (MDHHS).

Responsibilities*

The role of the PHE Community Outreach Coordinator is crucial in fostering collaboration and enhancing the quality of maternity care within the state of Michigan. By fulfilling these responsibilities, you will play a critical role in promoting high-quality maternity care and supporting the health and well-being of Michigan families.

Here are the key responsibilities you would be expected to fulfill:

Coordination, Outreach and Engagement:

  • Engage with clinical partners by establishing and maintaining strong relationships with across practice settings (e.g., outpatient clinics, hospitals)

  • Engage with community partners by establishing and maintaining strong relationships with community based organizations and hubs providing services in regions served by clinical partners

  • Lead implementation of QI activities linking clinics and community partners throughout the state.

  • Connect with statewide Perinatal Quality Collaboratives (PQCs), health departments, and multidisciplinary teams to advocate for and implement improved maternity care practices and policies.

  • Focus on enhancing the overall maternity care experience for Michigan families by promoting best practices and innovative care models in prenatal and postnatal settings.

  • Act as the main liaison for prenatal care clinics participating in quality improvement initiatives by providing presentations, support, guidance, and resources to facilitate their involvement and success in these initiatives.

  • Develop and execute communication strategies to ensure timely and effective dissemination of information to all stakeholders, including clinic staff, healthcare providers, and community partners.

  • Proactively identify and engage new clinics and potential partners to expand the reach and impact of quality improvement efforts.

  • Work in tandem with the OBI teams to ensure alignment with organizational goals and to leverage collective expertise and resources.

  • Organize and participate in meetings (virtually and in-person), workshops, and events to foster community engagement and to share knowledge and best practices with stakeholders.

  • Track the progress of outreach and engagement activities, and regularly report on deliverables per our funding requirements.

  • Support pilot sites in the launch and development of a social needs closed-loop referral process within obstetric care settings in Michigan. This will include understanding the patient experience through patient reported outcomes.

  • Refine and develop new support resources to facilitate implementation of prenatal/postpartum QI activities.

Other Duties as Assigned

Required Qualifications*

  • Associates Degree in Health Services, Public Health or related field.

  • Minimum of three years' experience with community engagement and outreach in multidisciplinary settings.

  • Background or experience working with diverse populations and marginalized communities.

  • Demonstrated experience related to diversity, equity and inclusion

  • Ability to work independently and effectively manage multiple competing/shifting priorities and deadlines with minimal direct supervision.

  • Excellent organizational and project management skills.

  • Excellent interpersonal and communication skills (both written and oral) and the ability to successfully work with multidisciplinary teams.

  • Strong expertise in public speaking and/or leading large group discussions and trainings.

  • Proficiency in Microsoft Office Suite (e.g. Outlook, Word, Excel, PowerPoint), Google Suites (e.g. Docs, Sheets, Drive), DropBox, and Zoom.

  • Ability to work cohesively in a team environment.

  • Ability to travel as needed within the state of Michigan.

Desired Qualifications*

  • Experience working in a clinical setting, specifically with regards to operations, billing and overall logistics.

  • Experience in managing billing processes, insurance protocols, and payment structures.

  • Skilled in creating, updating, and maintaining comprehensive databases.

  • Experience in women's health, obstetrics care, or maternal child health

  • Interest in reproductive health equity

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.
