Make a Difference

The Supervisor of Case Management is located at Community Hospital of Anderson. This position will lead activities of assigned staff by communicating and providing guidance toward achieving department goals. The Supervisor will promote department goals by selecting, motivating, and training staff. This position will work on well-defined issues and/or projects. The supervisor may be required to interpret application of policies, procedures, and technical methods for staff. This position may be required to cover a caseload.

Exceptional Skills and Qualifications

Applicants for this position should demonstrate the ability to adapt to a variety of situations, changes, and new learnings. Applicants can motivate and lead high-performing teams.

• RN: Graduate of an accredited school of nursing, a minimum of Associate's degree required.

• Social Worker: A Master’s degree in Social Work from an accredited college or university is required.

• RN: Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC)

• Social Worker: Licensed as a Social Worker (LSW) by the Indiana Professional Licensing Agency is required.

• Certified Case Manager (CCM) or Accredited Case Manager (ACM) or equivalent certification is preferred.

• 2 (two) or more years of experience working in a hospital setting is required.

• Case Management experience is strongly preferred.

• Coordinate, lead, prioritize, and/or manage multiple projects competently.

• Strong relationship-building skills with all levels of the organization.Independently resolve problems.
