Hiring Range: $17.17- $22.32

The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities: • Oversees primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care. • Provides primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician. • Monitors patient care through assessments, evaluations and/or patient records. Advocates for resources and removal of barriers. • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

Complexity of Work: • Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.

Personal Protective Equipment: • Follows Standard Precautions using personal protective equipment as required.

Education:

•High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.Must be able to speak, read, write and follow instructions in English.
