Job Description

Community Maternity Services (CMS), is an agency of Catholic Charities of the Diocese of Albany. CMS is a non-profit agency that has been serving the Capital Region for 50 years. CMS provides family-oriented and community-based programs through individualized social services and community education programs. We offer a continuum of care for adolescents, young parents and their children.

CMS is currently hiring a CASE PLANNER to work in our PREVENTION PROGRAM. The Case Planner will work with families in Albany County with children at risk of foster care placement.

JOB SUMMARY:

  • Provide case management, safety assessment, home-based parent education, service plan related transportation and crisis intervenion for parents and children at risk of foster care involvement as well as families whose children are in foster care and are planning for their return home.

Prevention Case Planner Minimum Salary: $19.00/hour

Prevention Case Planner Maximum Salary: $21.00/hour

Prevention Case Planner: full-time (35 hours/week) and requires flexible scheduling. On-call responsibilities apply

QUALIFICATIONS:

  • Bachelor's Degree

  • Valid NYS driver’s license and vehicle in good working order

  • Experience working in the child welfare field

  • Be able to walk up and down stairs; lift/carry up to 40 lbs.; be able to sit and/or stand for extended periods of time and demonstrate a full range of motion.
