We are currently hiring a Code Blue Case Manager to work in our Delaware, Otsego & Schoharie Counties agency. As the Blue Case Manager, you would be responsible for:

  • Temporary for the Warming Station Season (11/1-3/31)

  • Provide case management services to homeless individuals at the Warming Station, including intake, assessment, referrals, and service plan creation.

  • Establish regular contact with each household, either in person or by phone, to monitor progress and address barriers to goal achievement.

  • Offer direct services such as advocacy, coordination, referrals, transportation assistance, budgeting, and financial support related to housing.

  • Maintain up-to-date case files, document all case management activities, and track grant funding agreement targets.

  • Collaborate with Catholic Charities and other agencies, attend meetings and trainings, and ensure the confidentiality of client information in accordance with agency standards.

Blue Case Manager Minimum Salary: $30/hr

Blue Case Manager Maximum Salary: $30/hr

Blue Case Manager Schedule: M-F at the Warming Station from 5-10:30 pm

Qualifications:

  • Associate’s Degree with 3 years of experience preferred

  • HS Diploma and 5+ experience required.
