Certified Licensed/Allied Health Riverwood Healthcare Center Hospital Nursing Full Time (40 hours/week) Day Shift 8-430 About the Position: The hospital social worker will coordinate individualized discharge planning for patients in conjunction with family, the healthcare team, and outside agencies. This position will ensure that well-coordinated patient centered care occurs, and the patient is able to transition care safely and timely. Also assists with placement for behavior health, assisted living, nursing home and swing bed. This position cares for patients of all ages from infant to elderly population. What we are looking for: Graduate of an accredited school with a degree in social work. Current licensure within the State of Minnesota required. Assessing patients to understand their needs and providing support to them, their families, and others close to them. Participating in care team meetings and sharing information about discharge plans. Helping patients with Advanced Directives if requested, ensuring they understand and can express their medical preferences. Knowing and following rules for Medicare, skilled nursing facilities, and other healthcare programs. Connecting patients and families with community resources and financial aid options. Advocating for patients, protecting their rights, and keeping their personal information confidential under HIPAA. Writing and recording plans for patients leaving the hospital. Offering emotional support to patients and families during their hospital stay. Building positive relationships with patients, healthcare providers, and insurance companies.
