POSITION SUMMARY The RN Lead is an essential member of the primary care team providing professional nursing care for patients within the Riverwood Primary Care Clinics. The Registered Nurse (RN) will provide direct and indirect patient care following established policies, procedures consistent with industry standards and best practice. This role works collaboratively with others to restore health and promote wellness through coordinating comprehensive nursing care for patients of all ages, gender, economic and social status, and backgrounds. POSITION QUALIFICATIONS Education and/or Experience Graduate of an accredited School of Nursing with an associate degree or higher Minimum of (1) year professional nursing experience, clinic experience preferred Experience with Epic strongly preferred Licenses/Certificates/Registrations: Current, unrestricted RN Licensure within the State of Minnesota required BLS is required What We are Looking for: Provide clinical leadership and oversight in a primary care clinic setting, collaborating with multidisciplinary teams. Conduct daily huddles, engage staff in team-based care, and ensure seamless clinic operations. Serve as a first point of contact for staff, address issues, and manage urgent patient calls. Manage RN schedules, facilitate Advance Care Planning, and coordinate short-term care coordination. Assist with patient transfers, prescription requests, care inquiries, and quality improvement initiatives.
