Intake Coordinator

Arms Acres in Carmel, NY is looking for an Intake Coordinator to coordinate and facilitate admissions to Arms Acres. In this role, you will maintain communication with regional representatives, referents, patients and family members and assess referrals for appropriate placement.

PLEASE NOTE: Tues - Sat, 2- 10:30p (and Sat hours are 9-5:30p)

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Answer and respond professionally and promptly to phone calls and e-mails

  • Interviews prospective patients and collect referral information including referring agency or individual, patient demographics, medical, psychiatric, and substance abuse history and insurance

  • Request/obtain necessary medical or psychiatric records when indicated and obtain clearances when indicated

  • Able to verify insurance and utilize systems, including but not limited to, E-Pace, Medicaid eligibility site, etc.

  • Coordinate admit date, program, and transportation

  • Accurately enter all data in Meditech system

  • Follow up and confirm scheduled admissions

  • Explain pre-certification process

  • Inform patient/family regarding permissible and not permissible clothing and other items

  • Arranges private payment plans with patient/family. Obtains credit card information and records on form

  • Enter lost referrals in Meditech system with detailed information

  • Updates Meditech call log system for current referrals ongoing

  • Provide admission assistant coverage as requested

  • Maintain communication with regional representatives and referral sources. Notify both when scheduled admits do not occur as planned

  • Utilize discharge and Census information when scheduling patients

  • Participates in Performance Improvement Initiatives

  • Other duties as required

Requirements

QUALIFICATION REQUIREMENTS:

  • Licensed and appropriately credentialed Qualified Health Provider (QHP), such as CASAC, LMHC, LMSW, LCSW, RN or Master Level Social Worker or Psychologists required

  • Prior Intake job experience preferred

  • Substance abuse treatment, and admissions experience is required
