Job Description Summary:

In this position, you will collaborate with physicians and multidisciplinary team members to provide primary care to patients, encompassing assessment, treatment, care planning, and medical care evaluation. You will also offer physical and psychological support to patients, their families, and friends.

Qualifications for LPN:

  • Current Montana Licensed Practical Nurse (LPN) license required.

  • Current BLS certification required as applicable to assigned area(s).

  • Previous experience in a similar position preferred.

  • As applicable to assigned area(s):

  • Must have capability to connect to internet for daily data exchange.

  • To be eligible for the Driver portion of the position:

  • Current Driver’s License required.

  • Driving record for the previous thirty-six (36) months must not reflect any major driving offenses (i.e; DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).

  • In addition, personal vehicles used for LH business:

  • State law minimum liability coverage limits required.

  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Qualifications for RN:

  • Graduate of RN program from an accredited school of nursing required.

  • Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana; required.

  • BSN degree preferred.

  • Minimum of one (1) year experience in similar position within a medical setting (eg, acute care, clinic, or other hospital experience) preferred. Experience in assigned area(s) preferred.

  • Possess knowledge of professional nursing theory, medical terminology and principles of professional nursing practice to administer and evaluate patient care.

  • Healthcare Provider (HCP) CPR and/or BLS certification required.

  • Understanding of the confidentiality laws that govern the provider-patient relationship, including but not limited to, HIPAA guidelines.

  • Knowledge of equipment, supplies, and instruments used in an acute care area; simple routine clinical laboratory methods, universal blood and body fluid precautions preferred.

  • Knowledge of OSHA rules and regulations, established protocol for storing poisons, narcotics, acids, caustics, and flammable items; restriction imposed by various managed care carriers, various forms inherent to profession preferred.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

  • Exhibits a commitment to professionalism, civility, teamwork, and customer service while undertaking all tasks and interactions with others.

  • Exhibits knowledge of policies and procedures as they relate to the specific job duties, patient care and safety and as applicable to assigned area(s).

  • Exhibits knowledge of staffing standards and a demonstrated commitment to cost containment with all resources.

  • Operates and maintains equipment to departmental standards as applicable to assigned area(s).

  • Maintains nursing standards of care and executes appropriate provider orders. While executing patient care, exhibits knowledge of and maintains the patient’s right to diversity, privacy, and confidentiality.

  • As applicable to assigned area(s): Performs a standardized focused assessment or reassessment of patient at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.

  • Administers medications, treatment, and provider-specific care to patients in compliance with nursing policies/procedures, LPN scope of practice, and as applicable to assigned area(s).

  • Communicates all appropriate and critical changes in the patient’s condition, status, and findings to the appropriate stakeholders in a timely manner and as defined by policies and procedures and/or provider orders as applicable to assigned area(s).

  • Documents all clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements as applicable to assigned area(s).

  • Contributes to maintaining an updated, individualized plan of care that includes; goals, interventions, medications and visit frequency orders as applicable to assigned area(s).

  • Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.

This position offers the opportunity to work flexible shifts, including weekends and holidays, allowing for a dynamic and varied schedule. In addition to your regular shifts, you'll have the chance to attend meetings, educational classes, unit staff meetings, quality improvement or clinical case reviews, unit competencies, and certification courses, all of which contribute to your professional development and growth.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
